Job Description:
The Hospice Admission Nurse plans and delivers appropriate palliative & hospice care to patients through assessment, intervention, implementation, and evaluation while effectively interacting with patients, family members/significant others, and interdisciplinary team members to maintain standards of professional nursing and clinical competency.
Essential Duties & Responsibilities:
• Determine the appropriateness of referrals, coordinate and conduct
hospice pre-admission activities;
• Describe and explain hospice services to a potential patient/family and
elicit response(s) to questions regarding patient/family attitude(s) toward
hospice care, physical and psycho/social/emotional status in order to put
the patient and family at ease about choosing hospice and fully inform
them about the program;
• Gather pertinent information necessary for admission;
• Speak to patient/family about insurance coverage and financial obligations,
using knowledge of hospice fees, cost and reimbursement sources;
• Promote hospice services to ensure appropriate referrals to hospice
program to discharge planners, physicians, other health care providers and
referral sources;
• Manage care of patients, coordinate all services and facilitate
communication between all providers.
• Provide skilled nursing care and prescribed treatments to patients and
families in their home.
• Maintain productivity expectations.
• Provide documentation of services utilizing problem-oriented record.
• Plan for continuity of care with nursing team members, patient, family, and
other disciplines.
• Use opportunities for teaching nursing care, comfort measures and health
practices to the patient, families and others.
• Instruct and supervise care given by the Home Health Aides.
• Communicate and coordinate with other health and social agency
personnel to assist in planning, implementing, evaluation service to the
patient and family;
• Complete Hospice Item Set (HIS) time points. Accept accountability
for patient satisfaction scores and quality of care outcome measure
results;
• Initiate the plan of care, as assigned, and make necessary revisions as
patient status and needs change;
• Administer medications and treatments as prescribed by the physician;
• Counsel the patient and family in meeting patient needs related to physical,
emotional, and spiritual needs;
• Provide information to the patient and family regarding the dying process
and expectations.
What You'll Need:
• BSN strongly preferred.
• Current valid license to practice as a registered nurse in Massachusetts.
• Current Basic Life Support Certification
• Valid Massachusetts driver’s license
• 1+ year of recent professional nursing experience
• Hospice or end of life care experience preferred.
