Job Description:
Beth Israel Lahey Health at Home is looking for Weekend Baylor RNs who want to join our Home Health team! Here at BILHAH, we keep our travel territories small to ensure that patient care comes first. We are offering weekend Baylor (24/32 or 18/24) shifts for the following territories:
Danvers, MA and surrounding towns
Woburn, MA and surrounding towns
Watertown, MA and surrounding towns
*Please note that the orientation period for this position is 12 weeks long. During this time, the candidate may be asked to attend orientation sessions within the business week.
What You’ll Do:
The Weekend Baylor RN plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities in accordance with home care regulations and payer guidelines.
More Specifically:
- Assume responsibility to coordinate patient care for assigned schedule including but not limited to: appropriate delegation to LPN staff, patient condition change visits, and completion of OASIS time points. Accepts accountability for patient satisfaction scores and quality of care outcome measure results.
- Complete and document an initial assessment of patient and family to determine home care needs. Provide a complete physical assessment and history of current and previous illness(es.) Develop a patient specific plan of care, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Include the patient and the family in the planning process.
- Initiate the plan of care and make necessary revisions as patient status and needs change.
- Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician.
- Counsel the patient and family in meeting nursing and related needs.
- Respect and support patient and family’s cultural beliefs and values, individual needs, health goals and treatment preferences.
- Provide health care instructions to the patient as appropriate per assessment and plan of care.
- Instruct, supervise and evaluate home health aide care provided every two (2) weeks, as needed.
- Manage plan of care in accordance with evolving nursing care needs and continued eligibility for services.
- Identify discharge planning needs as part of the care plan development and is in communication with the primary RN Case Manager.
What You’ll Need:
- An active valid Massachusetts RN license;
- An active American Health Association BLS certification;
- Associate’s Degree or BSN (preferred) from an accredited school of nursing;
- Minimum of 1-2 years nursing experience, at least one of which is in the area of home care nursing with OASIS experience.
